This usually happens when the computer is unable to detect the Azeron controller.
Unplug and reinsert the cable in both ends and you'll probably get things up and running again.

1 plug the USB device into your PC.
2 Open the Device Manager by pressing Win + X and selecting Device Manager.
3 In the header menu of the Device Manager, click the Action tab and select Scan for hardware changes from the list.
4 Expand the Universal Serial Bus controllers section.
5 Right-click the Unknown USB Device (Device Descriptor Request Failed) driver and
6 select Uninstall device.
7 Restart your PC.

You can also try this video to reset the controller

It keeps getting stuck at the rest button point. Teensy stays greyed out and never changes colour no matter which USB port I choose.
It has always been hard to plug my cable into the USB could it be that the cable or the USB port is damaged?

Yes it could be a bad cable or a bad USB port
Have you tried to remove the software and uninstalling the Device Descriptor Request Failed driver as listed above?
Have to tried to right click on Teensy and run as admin?

Yes, same problem. I just noticed the computer end of the USB cable is bent; that must be it.
