Azeron Classic Thumbstick Module Replacement

I am attempting to order a replacement thumbstick module for my Azeron Classic from the following page:

Analog stick and 5 way button kit

However, I am unable to complete the form and add the product to the cart. My problem is with the "order number" field which is required. I did not order it myself. It was a gift from some years ago, so I can not supply the order number.

Please let me know what can be done to complete this order.
